Please use the form below to apply for the competition. Information from all five team members must be entered into a single form. Please do not attempt to fill out the form below until you have the information for all five team members. Note that this is a slightly different procedure than in years prior. Please reference the Team Composition section before applying. Participation in last year’s competition does not guarantee acceptance for the 2011 competition.
